Format Note

Each case study follows a fixed structure: Context (why this problem, why now) → Decision log (timestamped decisions with alternatives considered) → Evidence (test runs, data, failure analysis) → Retrospective (what the decision log reveals in hindsight). No conclusions are declared until the evidence section is complete.
